Understanding Direct Primary Care

What is Direct Primary Care?

Direct Primary Care (DPC) is a health care model where you pay your doctor a simple monthly fee for access to care whenever you need it. It means you get longer visits, direct communication with your doctor, and a more personal, stress-free healthcare experience without the hassle of insurance.

What is the difference between concierge and DPC?

Concierge typically has higher membership pricing and bills insurance. DPC has lower monthly memberships and does not bill insurance, but instead facilitates prior authorizations of medications and imaging with insurance companies.

Membership and Insurance

Do I need health insurance?

Fusella Family Medicine is NOT a substitute for health insurance. It is highly recommended you maintain health insurance to help pay for labs, medications, imaging, specialty care, hospital services, and more.

What if I do not have insurance?

Health insurance is not required for membership, although it is highly recommended. Membership at Fusella Family Medicine does not constitute health insurance. Having health insurance will help to pay for additional costs outside of your membership such as labs, medications, imaging, specialty care, and hospital services.

Can I use my HSA to pay for my monthly membership?

Yes! As of January 1, 2026, the passage of the One Big Beautiful Bill Act allows for DPC membership fees to be paid using your HSA. There are finer details and limitations on fee amounts, so we recommend consulting with your tax accountant or your employer about this first.

What if I need to cancel my membership?

We understand situations may arise that require cancellation of your membership. You are free to cancel your membership at any time. Please note, if you were to decide to return to our practice in the future, there is a re-enrollment fee.

Appointments and Access

Do you offer same day or next day appointments?

Yes. We offer same day or next day appointments.

Do you accept walk-ins?

Because we accommodate same day or next day appointments, walk-in appointments will not be accepted. We strive to devote time and attention to each patient concern, and this is best accomplished through scheduled appointment times.

What if Dr. Fusella is away on vacation?

Unless there is an emergency, all patients will be notified of a planned away time for Dr. Fusella. Patients will be given ample time to have their needs addressed prior to Dr. Fusella being away.

Medical Care and Services

What if I need to see a specialist?

If the need arises for you to see a specialist, we will help to facilitate such a referral within your health insurance network.

What if I need to use the Emergency Room or be admitted to the hospital?

While it is our goal to maintain your good health, certain situations may arise that require use of the ER or hospital admission. We will help coordinate such care and will follow up with you after discharge from the hospital. However, we do not provide ER or inpatient care.

What about vaccinations?

We recommend all age and condition appropriate vaccines and require all pediatric patients to be vaccinated. We do not provide vaccine waivers or medical exemptions. Although we are not able to offer vaccines in our office at this time, we are happy to coordinate vaccination through the local health department, pharmacies, or another office.

Do you perform home visits?

We will make special accommodations for members who are acutely ill and cannot otherwise come to the office. However, this is not included in the price of the monthly membership. If a home visit is necessary, an additional fee of $150 will be charged. We do not provide chronic care for homebound patients or nursing home residents.

Is there anything you do not do?

We do not offer obstetrical or prenatal care. We do not offer urgent care, emergency care, or inpatient care. We do not perform major surgeries. We do not provide chronic opioid or suboxone management. This is not a comprehensive list. If the need arises for more specialized care, we can help facilitate this on your behalf.

Policies and Emergencies

What is your cancellation and no show policy?

We understand that unexpected events arise. If you need to cancel or change your appointment, we request 24 hours notice. If you no-show an appointment or cancel within less than 24 hours, a $50 fee will be applied.
