Joining Fusella Family Medicine means you get a doctor who knows you, not just your chart. Your monthly membership gives you direct access to your physician, longer visits, same- or next-day scheduling, and ongoing support for your health.
We do not bill insurance of any kind.

No copays. No surprise bills. No insurance telling us how to care for you.

*Unfortunately, we are unable to accept children patients below the age of 6 at this time.

**PLEASE NOTE: If you choose to leave the practice and decide to rejoin later, there is a $400 re-enrollment fee, and re-enrollment will depend on availability at that time.

Meet Dr. Fusella (optional)

New to Fusella Family Medicine and Direct Primary Care? Feel free to schedule a meet & greet with Dr. Marissa Fusella before enrolling — though it’s not required.

Step 2

Get Enrolled

Please complete your enrollment by choosing a membership that fits you or your family. Enrollment is simple and handled through our secure online portal. 

Step 3

Patient Portal Access

Once enrolled, you’ll receive instructions to set up your patient portal. This will be our primary way to communicate, share documents, and manage appointments.

Step 4

Choose Your Membership Start Date

Please decide when you would like your membership to begin. You’ll have full access to Dr. Fusella as your DPC physician starting on day one of your chosen start date.

Step 5

Schedule Your First Appointment

Kindly schedule your initial visit within 30 days of your start date. This visit allows us time to review your medical history, goals, and create a plan tailored to you.

Step 6

Complete Required Documents

Legal and intake documents will be sent via secure e-signature. Complete them electronically, print and fill out, or arrive a few minutes early to finish in the office.

This is your ongoing cost. It covers your primary care, same or next day access, longer visits, and direct communication with your physician.

This is a one-time cost charged when you join Fusella Family Medicine. It covers your onboarding, new patient setup, and medical record review.

Frequently Asked Questions


Does Fusella Family Medicine accept Medicare?

Fusella Family Medicine has opted out of Medicare. This means that neither our practice nor our patients may bill Medicare for services received here. Medicare patients who choose to become members will be required to sign a private contract acknowledging this arrangement. If you have questions about how this affects your coverage, we recommend speaking with your Medicare plan administrator before enrolling.

Do you accept or bill insurance?

No. Fusella Family Medicine is a Direct Primary Care practice and does not accept or bill health insurance of any kind. Care is provided through a simple monthly membership fee paid directly to the practice. We recommend that all members maintain their own health insurance to cover labs, imaging, specialist visits, hospital services, and emergencies.

Does my insurance plan work with a Direct Primary Care membership?

It depends on your plan type. Patients with EPO or PPO insurance plans are generally well-suited for a DPC membership — these plans allow you to see out-of-network or non-participating providers without requiring referrals from a primary care physician.

Patients with HMO insurance plans may find this model more challenging. HMO plans typically require care to be coordinated through an in-network PCP, and referrals to specialists must come from that in-network provider. Because Fusella Family Medicine does not participate in insurance networks, we are unable to fulfill that role under an HMO plan. If you have an HMO, we encourage you to review your plan details or contact your insurer before enrolling.

Do I need health insurance?

Fusella Family Medicine is NOT a substitute for health insurance. We highly recommend keeping health insurance to help pay for labs, medications, imaging, specialty care, hospital services, or emergencies.

What if I don’t have insurance?

Health insurance is not required to become a member, but it is highly recommended. Membership at Fusella Family Medicine does not count as health insurance. Insurance still plays an important role in paying for services outside of your membership.

Can I use my HSA to pay for my monthly membership?

Yes! As of 1/1/2026, the passage of the One Big Beautiful Bill Act allows for DPC membership fees to be paid using your HSA. Please note: there may be limits on fee amounts, so we recommend consulting your employer or tax advisor.

What is the difference between concierge and DPC?

Concierge practices often have higher membership pricing and still bill insurance. Direct Primary Care does not bill insurance, and monthly membership fees are lower. We will still help facilitate prior authorizations for medications and imaging with insurance companies when needed.

What if I need to cancel my membership?

You may cancel anytime. If you choose to return in the future, there will be a re-enrollment fee.

Do you perform home visits?

We may make special accommodations for members who are acutely ill and unable to come to the office. This is not included in the monthly membership cost. If a home visit is necessary, the fee is $150. We do not provide chronic care for homebound patients or nursing home residents.

Is there anything you do not do?

We do not offer obstetrical or prenatal care. We do not provide urgent care, emergency care, or in-patient hospital care. We do not perform major surgeries and we do not manage chronic opioid or suboxone treatment. If you need more specialized care, we will help coordinate referrals.
